Bring check for $100 payable to MDWFP Foundation on day of training. FEE MUST BE PAID BY CHECK. NO CASH. BAI certification will not be awarded without $100 check to MDWFP Foundation. Required forms will be emailed to all participants after the registration window closes. Original copies of the forms, including all required signatures, should be provided, along with the check, to Judy Terry on the day of the training.
